Job Summary:


A paraprofessional who is specifically trained, certified and competent; performing assigned tasks and functions of personal care to the patient in various locations under the direction, instruction, and supervision of the registered nurse (RN).

Job Description:


Caring Hearts Home Care is looking for a few mature, committed and conscientious professionals to add to our professional staff of caregivers. We are currently looking for individuals who love what they do and are committed to enhancing the lives of others. Our current staffing needs are for employment at our assisted living facility and within our catastrophic services unit.

At our assisted living facility, we provide 24-hour service support for the residents who reside there. Typically support services are personal care, medication reminders, housekeeping support and companionship. At this site we require individuals who are willing to work within a team frame concept and support one another in the delivery of services to our residents; thereby ensuring the health, safety and welfare of the residents within our facility.

We are also looking for CNAs to work within our catastrophic services unit, where we provide care for individuals with severe physical impairments; some with closed head injuries. These individuals should have experience in dealing with spinal cord injuries and possess the commitment, compassion and thoughtful understanding to address the service needs of individuals whose lives have been turned upside down. Positions are currently available in the Southfield and Sterling Heights, Michigan areas.

All positions are immediately available for the right applicant. Starting pay is directly commensurate with skill set and experience.

Responsibilities:


  • Understand and adhere to established CHHC policies and procedures.
  • Perform personal care, bathing, showering and/or bed baths as ordered by the plan of care.
  • Complete appropriate visit records in a timely legible manner as per CHHC policy.
  • Report changes in the patient’s condition and needs to the office.
  • Perform household services essential to health care in the home as assigned.
  • Care of bathroom, kitchen, bedroom, and other areas as outlined in plan of care.
  • Laundry, meal preparation, dusting, vacuuming, mopping, light housekeeping, assist with ambulation, and exercises as assigned.
  • Perform simple procedures as an extension of the therapy services, e.g., range of motion (ROM) exercises as assigned.
  • Assist with medications that are ordinarily self-administered as assigned.
  • Attend patient care conferences as scheduled.
  • Be on time for assignments.
  • Respect and accept family values and cultural differences.

Personal care functions include:

  • Bathing (bed, tub, shower, or sponge bath) Skin Care Toileting (bedpan, urinal, commode and or toilet) Grooming (shampoo, nail care and amp; shaving) Oral Hygiene (denture care).
  • Assist with dressing and undressing.
  • Positioning and transferring clients as instructed.
  • Work within the plan of care at all times.
  • Good communication skills with clients, caregivers, and management team.
  • Assist with Activities of Daily Living (ADL’s).
  • Take vitals, temperatures, pulse, respiration and blood pressure as indicated in plan of care.
  • Maintain HIPPA compliance at all times.
  • Promote and encourage clients to be independent by cuing and prompting.
  • Assure safety and comfort at all times.
  • Understand Recipient Rights and Responsibilities.
  • Ability to effectively us a Hoyer
  • Ability to effectively use Glucose monitor Machine.
  • Knowledgeable about PACE policies and procedures.
  • Reporting abuse and neglect of a client to the office.

Qualifications:

  • Must meet Home Health Aide or Certified Nurse Assistant training competency.
  • Have a sympathetic attitude toward the care of the sick and elderly.
  • Ability to carry out directions, read, and write.
  • Maturity and ability to deal effectively with the demands of the job.

Requirements:
